Case Presentation

Mr. and Mrs. AB have been married for five years. Mr. AB is 31 years old, and Mrs. AB is 28. They did not start a family immediately after getting married in order to focus on their careers. However, they have been trying to conceive for the past two years without success. Mr. AB works with a pharmaceutical company, heading a unit that manufactures estrogenic vaginal creams. Mrs. AB is a corporate lawyer. You first met Mr. and Mrs. AB one year ago when the couple approached you, as their healthcare provider, with their reproductive health concerns.

On your initial examination, Mr. AB is 5 feet 10 inches (1.78 meters) and has a body mass index (BMI) of 31 kg/m2. Mrs. AB is 5 feet 4 inches (1.63 meters), and her BMI is 33 kg/m2. She confirmed menstrual irregularities for several years. You note Mrs. AB’s noticeable facial hair. Mr. AB’s semen analysis results show ejaculate volume of 1.1 mL and sperm count of 2 x106 spermatozoa/mL. The couple confirmed that they ate out regularly due to their busy work schedules and have not had time to use their gym membership.
