Case Presentation

Mr. AD is a 59-year-old African man with diabetes managed with metformin and high blood pressure managed with an angiotensin II receptor blocker. He presents with back pain and nausea and confirms that he has been taking ibuprofen to manage his pain.

On examination, Mr. AD’s feet are visibly swollen around his open-toe sandals. His body weight is 98 kg (216 lb), and his height is 1.8 m (5 ft 9 in), indicating a body mass index of 30 kg/m2; his heart rate is 73 beats per/minute, and his blood pressure is 97/66 mm Hg. On evaluation, his 6-hour urine output is 250 mL, and his serum creatinine level is 2.0 mg/dL. Liver function test results are normal.
